If your child will be entering from a school within the Mobile County School System. You must bring the following documents in order to register at Alba Middle School:

1.       Proof of residence (two)

2.       Your childs 2008-09 report card 

If your child is entering from another school district, you must bring the following documents in order to register at Alba Middle School:

1.       Birth Certificate

2.       Social Security Card

3.       Alabama Immunization Certificate(s) - This can be obtained from your physician or the local health department

4.       Proof of residence (two)

5.       Proof of Legal Custody (if student does not live with both biological parents.)

6.       Past grades/transcripts

7.       Name and address of prior school
